Dear Parents of Pacific School,
We hope you are all staying well and healthy. During this unprecedented time, we feel privileged to have you as a part of our Pacific School community. As we are all in an uncertain time together; we hope we can come together even if virtually or socially distanced to support each other. The Pacific School Foundation is a nonprofit run by volunteer parents of current students. It supports the school by raising vital funds to cover the difference between the
students' education and the amount the state gives to the school. We rely on the kind support of all Pacific School parents in any way that is possible. Fundraising this coming school year will be a challenge for us as we are unable to all come together for our typical fundraising events. We hope that classes are able to come together to raise the necessary funds to support the school. This will take leadership from each class. - Life Lab – Along with FoodLab, this school garden program teaches students Life Science and raises food used in the cafeteria.
- Music, Theater, and Visual Art – Parents’ Club pays for all of the costs to perform a school play every other year, and almost all of the costs of Orff music, instrumental music, and visual arts instruction. Pacific is one of few elementary schools in the county still maintaining an instrumental music program.
- School Library – Parents’ Club pays for staff, books, and supplies for the school library.
- Classroom Grants – Every teacher can apply to Parents’ Club for classroom grants for special supplies or projects.
You can also make contributions directly to these areas that are not a part of the Parent’s Club general fund:
- FoodLab – Seen as a national model, FoodLab teaches life skills, culinary arts, and reinforces the core curriculum. FoodLab has its own fund, supported by Friends of FoodLab, so if you would like your donation to go directly to FoodLab please make that clear on your check. All proceeds from the Farm-to-Table event do go directly to FoodLab.
- Fifth-Sixth Grade Field Trips – The fifth-sixth grade field trips, including the trip to the Balclutha and the Environmental Living Program (ELP), is funded by the independent fundraising efforts of the fifth and sixth graders.

2019-20 Back-to-School Forms Are Ready - All forms should be submitted to the school office or emailed to Registrar Chyna Darby, cdarby @ pacificesd.org. (Preschool forms here.)
1. Emergency Form - ALL STUDENTS must have a completed emergency form BEFORE attending school. The form is online. Parents sharing custody are each encouraged to complete separate emergency forms. You are welcome to use computers at the school to complete the form.
2. Free or Reduced Price Lunch Application (English or Spanish) and Application Instructions (English or Spanish) - ALL FAMILIES are requested to please complete this CONFIDENTIAL application. Doing so will not hurt the finances of the school and allows us to have accurate information about our student body when applying for grants and government programs. In past years, families who could qualify for free or reduced price ($0.40) lunches have gotten missed because they did not complete the paperwork - let's avoid that this year.
3. Notifications of Rights and Responsibilities English 2019-20 or Spanish 2019-20 - All families, please review. Then print, sign, and return the summary and signature page in English or Spanish.
4. Internet Acceptable Use Agreement (Spanish on second page) - Please submit for all students in grades 1 through 6. Not needed for preschool or kindergarten.
5. Field Trip Driver Form - You must complete this form at the beginning of the year to drive on field trips, even if driving only your own child. The form is now online. You are welcome to use computers at the school to complete the form. You will need to include copies of
- your drivers licence
- proof of insurance (the standard insurance card you keep in your car is fine)
- driving record from the DMV (available here: https://www.dmv.ca.gov/portal/dmv/detail/online/dr)
6. After School Care Application - for preschool and kindergarten. Please complete even if you do not plan to use the program in case you need to use it on an emergency basis. If you plan to use the program, include a prepayment of $75 or more. This is a fillable pdf. School Medication Authorization Form - Use this form to request designated school personnel to assist any student(s) who are required to take medication during the school day or on school field trips. Medication must be in original packaging with prescription or dosing information. Medication must be accompanied by the completed form with signed authorization from parent/guardian and health care provider.
Physician's Report for Kindergartners and New Students (English/Spanish) - This includes immunization records. Pacific Elementary School District adheres to all state immunization laws. For more information see www.shotsforschool.org.
Interdistrict Transfer Form (English or Spanish, PDF). Use this form only if you have been admitted to or are already a student at Pacific School.
Admitted interdistrict transfer students must complete a new inter-district transfer form EVERY YEAR BY FEBRUARY FIRST. Failure to do so could result in losing your spot at Pacific School. Mail or hand deliver the form to your HOME DISTRICT'S OFFICE, not Pacific School. Addresses are on page 3 of the form.

Request Special Meals Form (English or Spanish)- Use this form to request special meal accommodations for your student; submit to Pacific School Office.
School Medication Authorization Form - Use this form to request designated school personnel to assist any student(s) who are required to take medication during the school day or on school field trips. Medication must be in original packaging with prescription or dosing information. Medication must be accompanied by the completed form with signed authorization from parent/guardian and health care provider.
Physician's Report for Kindergartners and New Students (English/Spanish) - This includes immunization records. Pacific Elementary School District adheres to all state immunization laws. For more information see www.shotsforschool.org.
Oral Health Assessment Form - English and Spanish All families must visit the dentist, complete an oral health assessment, and submit the form during the student's first year in public school.
Interdistrict Transfer Form - Use the interdistrct transfer form only if you are a returning student or have been instructed to do so by the Pacific School Registrar.
Interest Form for Changing Programs - Use the appropriate interest form if you would like to change programs (Home Study, Independent Study, or Five-Day).
