2024 Back-to-School Forms - DUE TUESDAY, AUGUST 13
NOTE: Back-To-School Forms are now submitted through the Schoolwise Web Portal. Memo Subscribe If you haven't already, please subscribe to the school memo in order to receive important information in a timely manner. To do so, click here. Community and Volunteer Forms Community Networking - Please complete This Form to let us know what information we can share with other Pacific School community members, for carpooling, playdates, volunteer coordination, etc. Volunteer Agreements - Please complete the Volunteer Agreement 2024-25 if you volunteer in any capacity that involves observation of or engagement with students, such as driving for or chaperoning field trips, supervising the yard, or being a classroom volunteer. This includes driving anyone else's students to or from Friday IS/HS field trips. Field Trip Driver Required Documentation - Please complete the Field Trip Driver Form 2024-25 and the Volunteer Agreement 2024-25. Then please provide the following: copies of your driver’s license, proof of insurance (the card you keep in your car is fine), and driving record report from the DMV (available here for a $2 fee: https://www.dmv.ca.gov/portal/dmv/detail/online/dr). Bring these documents to the office or email them to Dona at [email protected]. All paperwork must be received a minimum of three school days before the trip. School Picture Order Form - 2024 Photo Order Form. School picture day is Thurs. 9/26, and the make-up day is Thurs. 10/3. Form and payment are due the day of the pictures, or you can return them earlier to the school office.
Bus-Walk-Bike-Scooter Form - to allow your student to walk, bike, scooter, or take the Santa Cruz Metro bus, to or from school independently, complete This Form, and submit it to the office. Please NO digital signatures on this form, and parents CANNOT sign for their student. Students (and parents/guardians) must review and genuinely agree to all of the information on the form.
Current Student Program Change Form - Available HERE - Use this form to request a change between the 5-day, independent study, and home study programs.
Request Special Meals Form (English or Spanish)- Use this form to request special meal accommodations for your student; submit to Pacific School Office.
School Medication Authorization Form - Use this form to request designated school personnel to assist any student(s) who are required to take medication during the school day or on school field trips. Medication must be in original packaging with prescription or dosing information. Medication must be accompanied by the completed form with signed authorization from parent/guardian and health care provider.
Report of Health Examination for School Entry (English/Spanish) - Required for any student entering the TK-6th CA public school system for the first time. This report includes immunization records. Pacific Elementary School District adheres to all state immunization laws. For more information see www.shotsforschool.org.
Oral Health Assessment Form - English Form or Spanish Form. All families must visit the dentist, complete an oral health assessment, and submit the form during the student's first year in public school. If, for any reason, you are unable to do so, please fill out the English Waiver Form or the Spanish Waiver Form.
Interdistrict Transfer Form - Use the Interdistrct Transfer Form only if you are a returning student or have been instructed to do so by the Pacific School Registrar.
Dientes Community Dental provides dental services to students on campus usually two times per school year. If you would like your student to participate, please complete This Form, and submit a paper copy to the office.
