Discipline, Incidents, and Workplace Injuries
Office Referral Form - Use this form whenever you bring a student to the office for any reason
Incident Report Form - Use to document any injury that isn't extremely minor and any incident that is significant or potentially significant. Complete this form, sign, and email to Eric and Hillary or put in Eric's office. Examples of times to complete the form: any incident which requires a trip to the doctor, any incident in which there is a concern about possible concussion, any incident which strikes you as significant or concerning, any incident which you can imagine becoming the topic of or involved in future litigation, etc. If you are on the fence, just fill out the form, it doesn't hurt. This can include incidents involving staff members, not just students.
Workplace Injuries - If any staff member is injured on the job, start by calling Company Nurse. They will help you through the process, including medical advice, forms, and referrals to an appropriate clinic if necessary. Also, please notify Eric and Elizabeth as soon as possible.
