Purchasing
For supply requests, use this link. For longer complex orders, email or speak with James. If you notice general school supplies are running low, enter a resupply request on the linked form. This list shows which accounts can pay for which types of items.

If you are doing the purchasing, download the appropriate form, complete, save or print, and email or give to Hillary, along with original receipts:
  • Reimbursement Request Form (Updated Nov 2025 - Please dispose of forms prior to this date)  Doing your own ordering and getting reimbursed is the easiest way for the school to process your purchase, as long as you know that the school has the budget for your items.
  • School checks can be issued (for example to a field trip venue), but must be requested TWO WEEKS ahead of time. Give your request to Hillary.
  • Purchase orders are also an option, talk to James.
  • School Credit Card Use Form (Updated Nov 2025 - Please dispose of forms prior to this date)  Please use the credit card as little as possible. If you do use it, you must get permission and GIVE HILLARY FORM AND RECEIPTS THE SAME DAY.

Independent Study
  • Student-Parent Agreements for Long-Term IS All long-term IS families should sign a paper copy of this document in the context of a meeting with the teacher within the first few weeks of school or the start of their long-term IS enrollment. 
  • Short- and Long-Term IS Written Agreement 2025 This is the written agreement that all students, 5-day and IS, will sign with back-to-school paperwork. It covers both short- and long-term IS situations. It's here for your reference. You will need to sign and file with the office these agreements for each student in your class. The office will help with the logistics. For long-term IS, these agreements must be dated before the first day of enrollment in the program.
  • Short-Term Independent Study Instructions (Spanish version) This is the all-purpose generic short-term IS assignment. You can supplement this assignment as much or as little as you want, depending on the student, length of absence, and other factors.
  • Student Short-Term IS Work Submission Form This is the form students should submit with their short-term IS packets. Long-term IS students could also use it for when they return from an extended absence. If work gets returned without this form, you can ask the student to complete it in class, or you can assess the work samples without it.
  • Short-Term IS Teacher Form This is the form you should complete for any short-term IS. Please complete it promptly and return it to Hillary with work samples, and the Student Work Submission Form if available. All delays make extra work for Hillary. Be quick!
  • Long-Term IS Teacher Forms This is the form you should complete for each long-term IS student in your class each attendance month. Make a copy of the form for your own use, you will not be able to edit the master copy. Be sure to follow the directions for work samples, etc. We are doing this entirely paper-based this year.

Field Trips
Field trip instructions for getting permission and clearing drivers for trips. Note: please start this process at least three weeks before your scheduled trip.
Teacher's field trip information form and proposed driver list. Turn driver list in to the office at least one week before your trip, and include a backup driver, or your trip may be cancelled.
Field trip driver roster. Keep a copy with you and turn a copy into the office the day of your trip.

Discipline and Incidents
Office Referral Form - Use this form whenever you bring a student to the office for any reason
Incident Report Form - Use to document any injury that isn't extremely minor and any incident that is significant or potentially significant. Complete this form, sign, and email to Eric and Hillary or put in Eric's office
Workplace Injuries - If any staff member is injured on the job, start by calling Company Nurse. They will help you through the process, including medical advice, forms, and referrals to an appropriate clinic if necessary.   • To register to receive emergency notifications, go to CruzAware. If you have a code red account, you can use that to log in. You can register both your work and home locations. You can also register for non-emergency alerts such as high surf advisories.

Call (831) 471-1121 Santa Cruz County Sheriffs Non-Emergency Dispatch If you see something suspicious, always document the incident by calling it in; the more that law enforcement hears from us, the more they will come here.

Leaves of Absence
Planned Leave Requests: Use the 2025-26 Staff Leave Request Form to request any planned absences from work. It is very helpful if you also use it for unplanned (emergency/injury/illness) leaves as well, even if you have to complete the form after the fact.

Late/Absent Notification: Any time you will be late or absent from work, you must email [email protected] and your closest coworker(s) to notify all relevant people who might be affected or need to know about your absence. If your emergency is such that you cannot email, please call the school office and follow up with an email when you are able to do so, even if it is after the fact.
